Click on a month and you'll be taken to a Google form to complete your appointment. Be sure to click SUBMIT when you finish. You are allowed to schedule one appointment per visit on the Google Form. If you need multiple appointments in a month, click 'Submit Another Response." Each partner may only schedule one visit per week.
